Officer Daniel Carter is just going about his normal shift when he comes across a figure covered in blood and hobbling down a stretch of road that is otherwise empty. He sends the young man to a small rural hospital that only has a skeleton team working there, only to find that the patients and employees are morphing into something horrible. As the dread grows more palpable, Carter directs the other people who have managed to escape the hospital on a perilous journey deeper into the hospital's subterranean bowels in a last-ditch effort to put an end to the nightmarish scenario before it's too late.
