Nica Pierce, who has spent the past four years in a criminally insane asylum, is mistakenly persuaded that she, not Chucky, murdered her entire family. But when her psychiatrist introduces a new therapeutic "tool" to facilitate his patients' group sessions— an all-too-familiar "Good Guy" doll with an innocent smile — a series of gruesome deaths begin to haunt the asylum, and Nica begins to question if perhaps she isn't insane after all. Andy Barclay, Chucky's adversary from the first three Child's Plays films who is now an adult, races to Nica's help. To save her, however, he must overcome Tiffany, Chucky's ex-wife, who will do everything, no matter how lethal or vile, to aid her beloved evil devil doll.
